What are the long-term effects of seizures on the brain? ANSWER Experts do not fully understand the long-term effects of seizures on the brain. In the past, most scientists thought that seizures did not cause any damage to the brain, attributeing brain damage to an individual to an underlying disease. Now, however, some doubts are beginning to emerge. Solomon L. Moshe, MD, director of Clinical Neurophysiology and Childhood Neurology at Albert Einstein College of Medicine in New York, is investigating the issue and remains cautious. "I don't think it's good to say one way or another if seizures do long-term damage," he says. "I think it all depends on the individual case." Moshe observes that children's brains are very flexible. Dr. Paul García, epileptologist and professor at the University of California, San Francisco, says he often hears a question from his patients: What are the long-term effects of seizures? Unfortunately, that question has no simple answer. Some research suggests that unchecked seizures can cause loss in visual memory, attention, problem solving and perception, while other studies suggest that the relationship of seizures with cognitive function is variable, because seizures are symptoms of an underlying condition. One thing is certain: Effects vary from person to person. "We all have patients who may have some seizures a year without significant cognitive effects," says Dr. John Betjemann, another UCSF epileptologist. "For them, the evidence of damage is not so clear." But Dr. Betjemann says he has other patients who don't stick too. Eventually, for a long period of time, they suffer from memory and cognition problems. While Dr. García recognizes these dangers, in practice the question of whether to worry about the theoretical effects of seizure can be "anaemia," he says. "There are undeniable physical risks of seizures, including injuries and deaths. In addition, seizures can interfere with driving, work and school." Worrying about what might happen doesn't feel like a productive strategy when patients can focus on their care and treatment. A factor makes a significant difference. According to Dr. Betjemann, the epileptic state, prolonged, uninterrupted or multiple seizures that occur very close, have subsequent effects. The Great Chicago Epilepsy Foundation says that every year, about 42,000 deaths and many more cases of brain damage follow episodes of status, and the epileptic status is the most likely way to cause long-term damage. Better control of seizures could be a key to improving long-term brain health. Some studies show the control of seizures may decrease the risk of cognitive damage. According to Dr. Betjemann, it has not yet been determined why, but this information suggests that the longer the remission, the better the prognosis.
